To apply for our sliding fee discount, a simple application must be completed and proof of income presented. Example of proof of income can include any combination of the following:

• Most recent 10-40 tax form

• Most recent W-2 form

• Most recent paycheck stub

• Most recent Social Security Statement

• A recent copy of Child Support Statement
   

 Applications can be processed at the time of the visit, if all applicable information is available including proof of income document(s).
