Welcome to Eagle View Community Health System
Eagle View Community Health System is a Federally Qualified Community Health Center (FQHC). The FQHC is a federal designation assigned to non-profit or public health care organizations that serve uninsured or medically underserved populations. FQHCs provide services to all persons regardless of ability to pay, and charge for services on a Board approved sliding-fee scale based on patients' family and income size.
Eagle View Community Health System is a proud member of both the Illinois Primary Health Care Association (IPHCA) and the National Association of Community Health Centers (NACHC) who work to bring community health centers from all backgrounds together to serve a common goal: to provide quality health care services to all, regardless of ability to pay.

As recipients of federal funds, all FQHCs must operate under a consumer Board of Directors governance structure. Eagle View Community Health System’s board of directors is comprised of qualified, compassionate individuals who are committed to making sure that every resident of Henderson, McDonough, Des Moines, Warren, and Mercer counties has access to quality and affordable medical, dental care, and behavioral health care.
ABOUT OUR CLINICS
Oquawka, IL is home to our primary clinic and administrative offices. This clinic opened in 1979 and offers medical, dental, behavioral health and substance care for adults and children. Several members of our Oquawka staff have been with us for many years and their dedication to healthcare is reflected in the community.
Our Stronghurst, IL clinic opened in 2000. The clinic offers medical, dental, and substance use care for adults and children.
In 2023 we welcomed our Monmouth, IL clinic location. This location is a brand new, state-of-the-art clinic that offers medical, dental, behavioral health and substance use services to those who reside in Warren County and beyond.
We are proud to have a dedicated team of medical providers, dentists, nurses, assistants, and other personnel working at our clinics. They are what makes us a success.
