Community Health Worker

The Community Health Worker assists individuals in accessing community resources, providing social support, providing basic health education, transporting individuals to and from medical appointments, and advocating for individuals and community health.

Essential Duties and Responsibilites

  • Establishes trusting relationships with patients and their families while providing general support and encouragement. 

  • Conducts comprehensive assessments of individuals, families, and groups, and provides ongoing follow-up with basic motivational interviewing skills. 

  • Conducts eligibility determination, enrollment, and follow-up assistance to help uninsured patients in gaining access to healthcare coverage. 

  • Encourages patient to attend all appointments and links to medical facilities as needed.  

  • Knowledgeable of community resources available and provide appropriate referrals to other social service agencies as necessary to meet patient needs. 

  • Maintains appropriate records of services to reflect patient interactions via documentation in the electronic medical record system or on appropriate encounter sheets. 

  • Coach patients in effective management of their chronic health conditions and self-care.  

  • Assist patient in understanding care plans and instructions. 

  • Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services. 

  • Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations. 

  • Primarily responsible for facilitating enrollment of Eagle View Community Health System patients and residents of Henderson, Knox, McDonough, Mercer, and Warren counties in Illinois and Des Moines County in Iowa, as well as consumers in West Central Illinois and Eastern Iowa and Northeast Missouri into affordable health insurance coverage through the Health Insurance Marketplaces, Medicaid or the Children’s Health Insurance Program (CHIP). Also encouraged to gain knowledge of related programs such as the Senior Health Insurance Program (SHIP). 

  • Conduct public education activities to raise awareness about affordable insurance options, including using health care insurance and about services available through a Federal Qualified Health Center (FQHC). 

  • After successfully completing federal and state training, maintain expertise in the Health Insurance Marketplace eligibility and enrollment rules and procedures; the range of qualified health plan options and insurance affordability programs; the needs of underserved and vulnerable populations; and privacy and security standards. 



  • Education: High school diploma or GED required. Associates Degree (Bachelor’s Degree preferred), in any of the following (or similar) disciplines: social work, speech communications, pre-law, business, marketing, education, English, public administration, health information.  

  • Work Experience: Knowledge of health plan programs and eligibility verification preferred.
